COVID Reporting Form graphicCOVID-19 Reporting Form Students and staff members who have a positive COVID diagnosis or are required to quarantine due to close contact should complete the COVID-19 Reporting Form. This form should be used instead of contacting a school principal, teacher, or school nurse. To complete the form in Spanish, click here.

Preventing the Spread of Germs

We encourage everyone to practice good hand and respiratory hygiene to help prevent the spread of germs, including:
  • Wash your hands often with soap and water for at least 20 seconds. If soap and water are not available, use an alcohol based hand sanitizer.
  • Avoid touching your eyes, nose and mouth with unwashed hands.
  • Avoid close contact with people who are sick.
  • Stay home and away from others when you are sick.
  • Cover your mouth and nose with the inside of your elbow when you cough or sneeze.
  • Clean and disinfect frequently touched objects and surfaces.

If your student or someone they have been in contact with is diagnosed with COVID-19, please follow guidelines from their medical provider and notify Pelham City Schools by completing the COVID-19 Reporting Form. If you have questions about symptoms your student is experiencing, please contact your medical provider for guidance.
